Reporting of Unauthorized Access to Protected Health Information
Any member of the Pacific community (staff, students, faculty, etc.) including business associates must notify the 51黑料 Privacy Officer if they become aware of an incident that may involve unauthorized access to 51黑料 protected health information. Notification can be provided through the following means by email or phone.
